29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  (a) 40 and 64 Mansion boulevard, roof of building -- On or prior to 10/18/18, anchor points for personal fall arrest and positioning system(s) were not installed in accordance with the manufacturer's instructions, in that the specified number and size of nails was not used.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  (a) 40 and 64 Mansion Boulevard, roof --  On or prior to 10/18/18, subcontractor employees did not use safety glasses while operating pneumatic nail guns during installation of roof shingles.  Relyea Sons & Daughters LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 968050, citation number 1, item number 2 and was affirmed as a final order on 6/16/14, with respect to a workplace located at 210 Church Street, Saratoga Springs, NY 12866.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) 40 and 64 Mansion boulevard, roof of building -- On or prior to 10/18/18, subcontractor employees did not use the fall protection system at all times while working on the roof of the building, exposing them to fall hazards of 10 to 25 feet.     Relyea Sons & Daughters LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 968050, citation number 1, item number 2 and was affirmed as a final order on 6/16/14, with respect to a workplace located at 210 Church Street, Saratoga Springs, NY 12866.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) 40 and 64 Mansion boulevard, roof of building -- On or prior to 10/18/18, subcontractor employees were not trained in the recognition and avoidance of fall hazards, including but not limited to:   safe installation and use of the vertical lifeline system, proper fit and safe use of the full-body harness, and difference between fall arrest and positioning device applications.    Relyea Sons & Daughters LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 968050, citation number 1, item number 4b and was affirmed as a final order on 6/16/14, with respect to a workplace located at 210 Church Street, Saratoga Springs, NY 12866.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)  64 Mansion Boulevard, porch roof area:   On or prior to 10/17/18, the ladder used to access the porch roof did not extend at least 3' above the landing area, exposing subcontractor employees to a 10 foot fall hazard.
