(a) 40 Mansion boulevard, roof of building -- On or prior to 10/18/18, anchor points for personal fall arrest and positioning system(s) were not installed in accordance with the manufacturer's instructions, in that the specified number and size of nails was not used.

G & Z Construction LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1155653, citation number 1,  item number 1 and was affirmed as a final order on 8/23/16, with respect to a workplace located at 1-A Lakeview Drive, Clifton Park, NY 12065.
