29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use: On or about October 15, 2018, on the roof of the building, outside wall, a 17-foot, fiberglass extension ladder was used to inspect a wall without the ladder being inspected, exposing the employee to a slipping hazard.
