The mold plate guard of the Patty-O-Matic automatic patty forming machine was removed, exposing employees to an amputation hazard.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about September 21, 2018, at the jobsite located at 5128 West Clinton Street, Tampa, FL. The mold plate guard of the Patty-O-Matic automatic patty forming machine was removed, exposing employees to an amputation hazard. The amputation of an employee's left middle finger tip was not reported to OSHA.
