29 CFR 1910.1200(e)(1): Employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) of 1910.1200 for labels and other forms of warning, safety data sheets, and employee information and training will be met: (a) TJ Fats, LLC dba Fat Shack, at 1635 8th Ave, Greeley, CO 80631: On and preceding 10/18/2018, the employer did not develop, implement or maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g) and (h) of 29 CFR 1910.1200 for labels and other forms of warning, safety data sheets, and employee information and training will be met. Employees used chemicals to wash dishes and for cleaning surfaces. This condition exposed employees to chemical hazards associated with use of chemicals such as, but not limited to: 1. Lemon Fresh Disinfectant Cleaner; 2. Keystone Multi-Quat Sanitizer; 3. Floor Cleaner & Degreaser. TJ Fats, LLC dba Fat Shack was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(e)(1), which was contained in OSHA inspection number 1329034, citation number 1, item number 1a and was affirmed as a final order on 8/9/18, with respect to a workplace located at 1635 8th Ave. Greeley, CO 80631.

Employees used chemicals to wash dishes and for cleaning surfaces. This condition exposed employees to chemical hazards associated with use of chemicals such as, but not limited to: 1. Lemon Fresh Disinfectant Cleaner; 2. Keystone Multi-Quat Sanitizer; 3. Floor Cleaner & Degreaser. 29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. (a) TJ Fats, LLC dba Fat Shack, at 1635 8th Ave, Greeley, CO 80631: On and preceding 7/11/2018, the employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Employees used chemicals to wash dishes and for cleaning surfaces. This condition exposed employees to chemical hazards associated with use of chemicals such as, but not limited to: 1.
