B&B Construction, LLC jobsite located in Lorain, OH:  On or about October 19, 2018, the employer did not ensure safety and health programs were maintained that cover job site hazards such as fall protection, PPE, and ladder safety.

B&B Construction, LLC site located in Lorain, OH:  On or about October 19, 2018, the employer did not ensure employees installing new roofing materials on a single story residential roof with a 4:12 pitch were protected from fall hazards of 10 feet 1 inch.

B&B Construction, LLC jobsite located in Lorain, OH:  On or about October 19, 2018, the employer did not ensure employees exposed to eye injuries while operating pneumatic nail guns during shingle installation were wearing safety glasses to protect against eye injuries.
