29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. On or about October 13, 2018, located in the INF Department on the Grinder Line at Amsted Rail in Groveport, Ohio, lockout/tagout procedures were not utilized when employees were changing out a pneumatic cylinder on FR-75 unpender and roller pins from FR-64 pneumatic upender, thereby exposing the employees to caught-in hazards.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about October 13, 2018, located in the INF Department on the Grinder Line at Amsted Rail in Groveport, Ohio, employees were exposed to machine caught-in hazards when removing roller pins from, FR-64, a pneumatic upender. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. On October 13, 2018, employees were removing the pins on FR-64 upender when one of the employees tripped an electronic photo-sensing eye causing the machine to actuate while an employee was at the location of its pneumatic ram causing crushing injuries to the employee.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. a. On or about October 13, 2018, located in the INF Department at Amsted Rail in Groveport, Ohio, the employer did not ensure employees were protected from machine caught-in hazards while performing a group lockout/tagout activity when removing roller pins from FR-64, a pneumatic upender. Three employees were involved in the pin removal without their locks affixed to energy isolating devices.

Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. a. On or about October 13, 2018, located in the INF Department on the Grinder Line at Amsted Rail in Groveport, Ohio, the employer did not ensure all authorized employees performing a cylinder change on FR-75 verified the isolation and deenergization of the upender.
