29 CFR 1910.272(e)(1)(ii): Current employees and new employees prior to starting work in the grain handling facility were not trained in the specific procedures and safety practices applicable to their job tasks including but not limited to cleaning procedures for grinding equipment, clearing procedures for choked legs, housekeeping procedures, hot work procedures, preventive maintenance procedures and lock out tag out procedures.    On or about July 28, 2018, an employee entered the tunnel under bin number seven to close the unload gate and the entrant was not trained on the specific lockout procedures that would be used to isolate the energy sources for the unguarded unloading conveyor.

Such locks and tags shall be removed in accordance with established procedures only by the employee installing them or, if unavailable, by his or her supervisor:  On or about July 28, 2018, an employee entered the tunnel under bin number seven to close the unload gate and the lockout procedures did not clearly address the use of locks to isolate the unguarded unloading conveyor and the entrant did not lock out the equipment or have control of the energy source.
