29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition:       (a) On or about October 17, 2018 - Aisle near restrooms, employees were exposed to slip and struck-by hazards when there was water on the floor in the aisle next to the restrooms causing a slip hazard for pedestrians and a struck-by hazard for pedestrians nearby when a forklift drove through the water.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) On or about October 17, 2018 - Presses with interlocked back gates, employees were exposed to struck-by hazards when lockout was not used when performing work such as, but not limited to, loading a roll of metal, changing robot suction cups, and cleaning out scrap/trash.  Employees used a tag when unplugging the interlock on the gates.
