On or about October 16, 2018, during the installation of steel decking material at a new commercial construction site located at 8247 Pittsburg Avenue N.E., North Canton, Ohio, the employer did not ensure that every employee working in a controlled decking zone (CDZ) had received the requisite training for working in such a CDZ.

Canton, Ohio 44720; the employer did not ensure portable ladders extended 3 feet above the upper landing surface where employees were exposed to fall hazards from working surfaces with a height of approximately 18 feet.
