29 CFR 1904.4(a): Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that: 1. is work-related per 29 CFR 1904.4(a)(1); and, 2. is a new case per 29 CFR 1904.4(a)(2); and, 3. meets one or more of the general recording criteria of �1904.7 or the application to specific cases of ��1904.8 through 1904.12 per 29 CFR 1904(a)(3). Paleteria Y Neveria La Michoacana store at 300 S. McLean Blvd., Elgin, IL 60123 - on or about October 22, 2018, the employer failed to record a work-related injury that occurred on September 16, 2018, and did not maintain required OSHA 300 form "Log of Work-Related Injuries and Illnesses." Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. Paleteria Y Neveria La Michoacana store at 300 S. McLean Blvd., Elgin, IL 60123 - on or about October 22, 2018, the employer failed to use and maintain required OSHA 300, 300-A, and 301 forms to record a work-related injury from September 16, 2018. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
