29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.    Location: 313-321 Pine St, Jersey City, NJ.    a.) Employees were exposed to electrical hazards when utilizing electric powered hand tools that were energized by an extension cord without a ground fault circuit interrupter, as well as not providing the employees with an assured equipment grounding conductor program; on or about 10/4/18.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.
