Street, NW, Fourth Level - On or about October 9, 2018, the employer contacted the Baltimore/Washington Area office of OSHA to report the hospitalization of an employee that occurred on or about September 14, 2018.

St., NW - On or about 10/22/2018, the employer failed to provide copies of the requested injury and illness records for calendar year 2014 through 2018 to an authorized OSHA representative.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.68(e)(3): The certification record required to be kept of each inspection which includes the date of the inspection, the signature of the person who performed the inspection and the serial number, or other identifier of the manlift which was inspected, was not made available to the Assistant Secretary of Labor or a duly authorized representative:   a)  325 7th. St., NW - After requests were made on October 22, 2018 and March 13, 2019, no certification records of monthly inspections performed on a manlift were not produced by this employer.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
