29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment was not determined on the basis of the following considerations: (a) This violation occurred on or about October 10 2018, at the job site located at 2050 N. Stemmons Freeway, Dallas, TX of the Dallas World Trade Center on the first floor; where the flexible cord to a Galaxy floor machine was not inspected prior to use exposing employees operating the unit to an electrical hazard.

29 CFR 1926.404(b)(1)(iii)(C): Assured equipment grounding conductor program. The employer did not establish and implement an assured equipment grounding conductor program on construction sites covering all cord sets, receptacles which are not a part of the building or structure, and equipment connected by cord and plug which are available for use or used by employees. This program did not comply with the following minimum requirements: (a) This violation occurred on or about October 10, 2018, at the job site located at 2050 N. Stemmons Freeway, Dallas, TX at the Dallas World Trade Center on the first floor where employees operating a Galaxy floor machine where the cord had a strain relief and missing a ground pin were exposed to an electrical hazard.
