29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Jobsite: One of employees working/walking in and around the "material" drop zone/area from the roof was not wearing protective helmet/hard hat. On or about October 10, 2018, the employee was struck in the head by a wooden pallet that was thrown from the 35-foot high roof, and the employee sustained a laceration to the head from the incident.

29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s): Jobsite: An enclosed chute of wood or equivalent material was not used to establish and maintain an appropriate safe drop zone/area on the ground where materials, such as (but not limited to) wooden pallets and plywood were thrown from the 35-foot high roof while employees were working/walking on the ground in and around the drop zone/area. On or about October 10, 2018, one of employees was struck in the head by a wooden pallet and sustained a laceration to the head from the incident.
