29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). South facing wall: Pump-Jack scaffolding contacted a 25kV - 3 phase distribution line while being dismantled where it was erected, used, and dismantled approximately eight (8) feet from the conductor exposing employees to electric shock hazards.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: South facing wall: Employees were using pump-jack scaffolding that had not been erected or dismantled by a competent person to install siding on a residential home. The scaffolding was not secured at the bottom and was approximately eight (8) feet from a 25kV - 3 phase distribution line exposing employees to falls, struck-by, and electric shock hazards.
