29 CFR 1910.132(a): Protective equipment was not used and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards encountered in a manner capable of causing injury or impairment in the function of any part of the body: a) Erie Bronze & Aluminum, melting operations. On or about October 19, 2018, employees performing melting operations were not required to wear hoods/ neck protection designed to prevent thermal burns during the pouring operations. Employees did not utilize neck protection while performing activities that included, but were not limited to, stirring and pouring molten from the ladle.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment: a) Erie Bronze & Aluminum, melting operations. On or about October 19, 2018, the employer did not have a certified hazard assessment that identified the proper PPE to be worn by employees performing the melting operations.
