29 CFR 1926.150(a)(1): The employer did not develop and maintain a fire protection program to be followed throughout all phases of construction and demolition work, and/or did not provide for the firefighting equipment as specified in this subpart:    (a) On the 3rd floor:  On or about 11/1/2018, the employer did not have an effective fire protection plan, in that sparks from operations on the 5th floor were allowed to fall onto debris on the 3rd floor, causing the debris to smolder.

29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work on, had the strength and structure integrity to support employees safely:    (a) On the northwest corner of the 3rd floor:  On or about 10/22/2018, the employer did not ensure that the walking/working surface on which the workers were working had the strength and structural integrity to support them safely, in that the floor was loaded with scrap concrete and construction debris, causing it to collapse down to the 1st floor.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    (a) On the 5th floor:  On or about 11/1/2018,  employees on walking/working surfaces were exposed to unprotected holes in the floor, which were approximately 12-15 feet above the next lower level.    (b) On the 4th floor:  On or about 11/27/2018, two employees on the walking/working surface were observed working at the edge of open holes in the floor without a form of fall protection, exposing the employees to a fall hazard of approximately 12-15 feet.

(b) On the 3rd floor:  On or about 11/27/2018, an employee misused a roofing bracket attached to a round column, which is not what it was designed to be used for.   (c) On the 2nd floor:  On or about 11/27/2018, an employee misused a roofing bracket attached to a round column, which is not what it was designed to be used for.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) On the 5th floor:  On or about 11/1/2018, the employer did not provide fall protection training to the employees exposed to fall hazards throughout the worksite, in that they issued fall protection to employees and didn't perform fall protection training until 10 days after the inspection had been opened.
