29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access: On or about October 24, 2018, employees performing siding installation from a pump-jack scaffold were exposed to a fall hazard of approximately 18 feet when the employer failed to ensure that a ladder was provided for access to the work platform. No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about October 24, 2018, at the subject jobsite, employees performing siding installation from a pump-jack scaffold were exposed to a fall hazard of approximately 18 feet when the employer failed to ensure that guardrails were installed at the ends of the work platforms and failed to install a mid-rail along the length of the platform. No abatement certification or documentation is required for this item.
