29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screwed: (a) On or about October 24th, 2018 and at time prior to that date, for the employees exposed to electrical shock hazards from an electrical extension cord attached to a generator where the strain relief has been damaged in the vicinity of East Thayer Avenue & North First Street in Bismarck, North Dakota.
