29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. Location - 2 Bruce Street, Littleton, MA On or about October 22, 2018, a frequent and regular inspection of the job site was not performed to protect workers from the following but not limited to hazards; 1. Fall hazards of greater than 21-feet while working from the roof without fall protection; 2. Workers operated the Lull, model 644B-42 powered industrial truck without receiving training.
