29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a.  On or about 10/24/2018, at the jobsite, the supervisor and subcontractor employees were installing clay bricks and foam board while working from a 3 tiered tubular welded scaffolding that was not fully planked on the working levels.

29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm):  a.  On or about 10/24/2018, at the jobsite, the wood planks on the 3 tiered tubular welded scaffold did not extend the required 6 inches past the centerline of the scaffold support beam.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:   a.  On or about 10/24/2018, at the jobsite, the wood planks on the 3 tiered tubular welded scaffold the planks on the top working tier were not secured in place and the planks were over 10 feet long and the ends extended a few feet past the frame support beam.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:  a.  On or about 10/24/2018, at the jobsite, employees were not protected from falling while working from a tubular welded scaffold. There were no guardrails on the ends or sides of the scaffolding.
