29 CFR 1926.1053(a)(1)(ii): Non-self-supporting portable ladder(s) were not capable of supporting without failure, at least four times the maximum intended load:  a)  At 31661 Spoonbill Rd. Spanish Fort, Alabama:  On or about October 24, 2018 and at times prior; the employer allowed his employees to use a job made ladder with one rail and no blocks installed exposing his employees to a fall hazard of 9.1 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  At 31661 Spoonbill Rd. Spanish Fort, Alabama:  On or about October 24, 2018 and at times prior; the employer allowed his employees to use a job made ladder on site and the rails of the ladder did not extend three feet above the landing to provide safe access exposing his employees to a fall hazard of 9.1 feet to the ground below.

Spanish Fort, Alabama:  On or about October 24, 2018 and at times prior; the employer allowed his employees to perform roof activities without fall protection exposing his employees to a fall hazard of 9.1 feet to the ground below.   THE SALVADOR ALEJANDRE BAUTISTA DBA SALVADOR ALEJANDRE BAUTISTA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1313629, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 30, 2018, WITH RESPECT TO A WORKPLACE LOCATED AT 24490 KIPPLING CT., LOT 187 BELLATON, DAPHNE, ALABAMA.
