29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  a.  On or about October 25, 2018, at the above addressed jobsite, employees performing carpentry operations from a Bil-Jax aerial lift, approximately seven (7) feet above the ground, were not wearing full body harnesses, thereby exposing the employees to falls.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. On or about October 25, 2018, at the above addressed jobsite, employees performing carpentry operations from the roof were exposed to falls, of twelve (12) feet, created by the presence of unprotected sides.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a. On or about October 25, 2018, at the above addressed jobsite, an employee conducting roofing operations, greater than twenty-five (25) feet above the ground, was not protected from falling.
