29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  1) Dock Area: On or about October 25, 2018, the employer exposed employees to a fall hazard of 4 feet in that all open sides around the docks were not guarded while employees were working around the dock and operating a forklift truck.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    1) Warehouse Area: On or about October 25, 2018, the employer exposed an employee to struck-by hazards by not certifying or evaluating employees operating the Toyota forklift truck model 80580 and serial 7BPUE15.
