29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:        a) 15 Prince Street, Warwick, NY - On or about October 24, 2018, an employee was working in a trench at a depth of 6-8 feet and had never received training about the hazards of working in or near open trenches.  The trench caved-in resulting in the hospitalization of the employee.
