29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: On or about October 2, 2018, jobsite located at 17250 SW Railroad Avenue, Indiantown, Florida 34956, the employer did not develope and implement a written hazard communication program for hazardous chemicals, for employees working with hazardous chemicals, including but not limited to, Dry Caustic Potash (containing 80-90% concentration of Potassium Hydroxide Flakes, a corrosive), and Biostable (R) 501 (containing 100% concentration with a pH of 10.7), Purolite PD206, Glycerin, Biodiesel (B100), and Methanol.

29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. On or about October 2, 2018, at the jobsite located at 17250 SW Railroad Avenue, Indiantown, Florida 34956, the employer did not establish and implement a written respiratory program for employees who were performing mixing operations and who were required to wear 3M Half Mask Elastomeric Respirators.

29 CFR 1910.134(g)(1): The employer permitted respirators with tight-fitting facepieces to be worn by employees with facial hair that came between the sealing surface of the facepiece and the face. On or about October 25, 2018, at the jobsite located at 17250 SW Railroad Avenue, the employer allowed an employee to wear 3M Half Mask Elastomeric Respirator while having facial hair that prevented a tight facial seal of the respirator.
