29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems, safety net systems, or personal fall protection system, such as a personal fall arrest, travel restraint, or positioning system. On or about October 25, 2018 and times prior thereto: a. Where employees in the Heat Treat Area were exposed to a fall hazard exceeding 4 feet when changing filters on Furnace #1 without a fall protection system. b. Where employees in the Heat Treat Area were exposed to a fall hazard exceeding 4 feet when changing filters on Furnace #2 without a fall protection system. c. Where employees in the Heat Treat Area were exposed to a fall hazard exceeding 4 feet when changing filters on Furnace #3 without a fall protection system. d. Where employees in the Heat Treat Area were exposed to a fall hazard exceeding 4 feet when changing filters on Furnace #4 without a fall protection system.
