On or about October 24, 2018, production shop, at the following resistance welding machines adequate water drainage and dry standing places such as false floors, platforms or mats were not provided:  a.  D resistance welding machine, b.  A resistance welding machine, c.  C resistance welding machine and, d.  E resistance welding machine,     Employees were exposed to slip and trip hazards where water accumulated at the floor.

On or about October 24, 2018, production shop, suitable dust collectors were not provided for the D, A, C, and E sanding machines in that the dust collectors were located inside the building, the dust collectors have no gauges or manometers to determine if the filters need replacement, the dust collectors lack explosion protection such as deflagration explosion vents, and the dust collectors were not bonded to the sanding machines and grounded.  Employees were exposed to dust explosion, deflagration, or other fire hazards from carbon steel tubing and pipe sanding dust.
