29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closedEmployer failed to ensure that unused openings in electrical enclosures were effectively closed. This was mot recently documented at 4404 S. 134th St Omaha, NE 68137. A junction box with open conduit knock-out was unprotected (bottom right location). Additionally, a high voltage electrical cabinet was observed having 8 machined holes that were not covered or protected otherwise. This junction box, and High Voltage electrical cabinet both located near lock out tag out station.
