29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about October 25, 2018, employees were exposed to a fall of approximately 9 feet while performing framing activities on a new commercial restroom facility located at 1500 Clague Road, Westlake, Ohio 44145.  The employees were walking the top plate of the building and the employer did not ensure that they were using fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about October 25, 2018, employees were exposed to a fall of approximately 9 feet while performing framing activities on a new commercial restroom facility located at 1500 Clague Road, Westlake, Ohio 44145.  The employees were walking the top plate of the building and the employer did not ensure that they were trained in how to recognize and minimize fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.      On or about October 25, 2018, employees were exposed to a fall of approximately 9 feet while performing framing activities on a new commercial restroom facility located at 1500 Clague Road, Westlake, Ohio 44145.  The employees were using an extension ladder to access the upper level and the employer did not ensure that it extended at least three feet above the landing area.
