29 CFR 1926.62(d)(1)(i): Each employer who has a workplace or operation covered by this standard shall initially determine if any employee may be exposed to lead at or above the action level.  On October 25, 2018, employees tearing off the bell tower on an old school building were exposed to lead when the building had not been assessed for lead or other environmental hazards prior to the work being performed.

29 CFR 1926.62(g)(1): "Provision and use". Where an employee is exposed to lead above the PEL without regard to the use of respirators, where employees are exposed to lead compounds which may cause skin or eye irritation (e.g. lead arsenate, lead azide), and as interim protection for employees performing tasks as specified in paragraph (d)(2) of this section, the employer shall provide at no cost to the employee and assure that the employee uses appropriate protective work clothing and equipment that prevents contamination of the employee and the employee's garments such as, but not limited to:  29 CFR 1926.62(g)(1)(i): Coveralls or similar full-body work clothing;  29 CFR 1926.62(g)(1)(ii): Gloves, hats, and shoes or disposable shoe coverlets; and  29 CFR 1926.62(g)(1)(iii): Face shields, vented goggles, or other appropriate protective equipment which complies with 1910.133 of this chapter.  On October 25, 2018, employees were exposed to lead while tearing down a bell tower on an old school when the employer did not provide protective clothing and PPE.

29 CFR 1926.62(i)(2)(iii): The employer shall assure that employees do not leave the workplace wearing any protective clothing or equipment that is required to be worn during the work shift.  On October 25, 2018, employees were exposed to lead while tearing down the bell tower of an old school when the employer did not ensure employees changed clothes before leaving the site at the end of the day.

29 CFR 1926.62(l)(1)(ii): The employer shall train each employee who is subject to exposure to lead at or above the action level on any day, or who is subject to exposure to lead compounds which may cause skin or eye irritation (e.g., lead arsenate, lead azide), in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program.  On October 25, 2018, employees were exposed to lead while tearing down a bell tower on an old school when they had not been trained to understand the hazards associated with lead or to follow lead safe work practices for lead abatement work.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  On October 25, 2018, employees were exposed to falls while working from a JLG Boomlift when they were not using lanyards to connect their harnesses to the lift.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On October 25, 2018, employees were exposed to falls while tearing down a bell tower on an old school when they were not using complete personal fall arrest systems or other methods to prevent falls.
