On or about October 25, 2018, employees were exposed to a fall of approximately 12 feet while performing framing activities on a new residential home located on SL 114 Atlantic Ave., North Ridgeville, Ohio 44039.  One employee was being lifted to upper levels on a JLG forklift and the employer did not ensure that the modified platform was being used as the manufacturer intended.
