29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. a) Ford Stamping Plant, Waste Management Building - On or about October 24, 2018, the employer did not ensure that each employee who was assigned to operate the Hyster sit down forklift (serial numbers N005V02443E) received practical training and was evaluated on their performance in the workplace. Employees were thereby exposed to struck-by and crushing hazards. In that the load engaging means were not fully lowered and the brakes were not set when forklift was dismounted and the operator was within twenty-five feet of the truck.
