29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. This violation was observed on or about October 23, 2018 at 9840 E. 81st St., Tulsa, Oklahoma, the employer did not ensure employees moving materials on the roof were provided with fall protection exposing employees to the hazards of falling approximately 20 ft. to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. This violation occurred on or about October 23, 2018, the employer did not ensure employees engaged in roofing work activities and who were working at a height greater than six (6) feet above ground level, were provided fall protection training. This exposed the employees to fall hazards.
