This violation was observed on or about October 23, 2018 at 9840 E. 81st  St., Tulsa, Oklahoma,  the employer did not ensure employees moving materials on the roof were provided with fall protection exposing employees to the hazards of falling approximately 20 ft. to the ground below.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. This exposed the employees to fall hazards.
