29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee working from an aerial lift: On or about October 24, 2018, employee was changing the direction of enclosure of fiber optics while operating an aerial lift without the use of fall protection equipment. This condition exposed employee to falls in excess of 10 feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about October 24, 2019, employer did not provide safety training to employee operating an airlift. This condition exposed employee to fall hazards exceeding 10 feet.
