On 10/11/2018, employees of Eduar Benitez, DBA Eduar Exteriors LLC were working on a residential building located at 2037 Waters Edge Court, Lake Saint Louis, MO.  Employees were installing shingles approximately 9 to 25 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection.
