29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a) Worksite - On or about October 26, 2018, employees were exposed to struck-by hazards to the eyes due to the employees using and working around pneumatic nail guns while on the roof without wearing safety glasses, and employees working on the ground cleaning up debris coming down from the roof without wearing safety glasses.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Roof at Worksite - On or about October 26, 2018, employees were exposed to fall hazards when working on a 10 on 12 pitched roof approximately 15 feet in height from ground to eave of roof without fall protection.  The employer did not provide training to the employees on fall protection.  b) Scaffold at Worksite - On or about October 26, 2018, an employee was exposed to fall hazards when working on a ladder jack scaffold approximately 15 feet in height from the ground without fall protection.  The employer did not provide training to the employee on fall protection.
