On or about October 26, 2018, jobsite located at 1610 Southern Blvd., West Palm Beach, Florida 33406, employees were exposed to a struck-by eye injury, when using a pneumatic nail gun to install roofing materials were not wearing eye protection equipment. REPEAT: Fastage Roofing Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1), or its equivalent standard, which was contained in OSHA inspection number 981870, citation number 01, item number 001, with respect to a workplace located at 421 SW Federal Highway, Stuart Florida 34994 and was affirmed as a final order on 08/11/2014.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about October 26, 2018, jobsite located at 1610 Southern Blvd, West Palm Beach, Florida 33406, employees performing roofing work on the low-slope roof with unprotected edges were exposed to approximately 18.4 feet in height without being protected from falling.
