On or about October 29. 2018, front parking lot, adequate training on the aerial lift operators manuals was not provided to an employee who operated aerial lifts to access or descend from the building roof. The employee was exposed to falls greater than 20 feet to a lower level.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a.  On or about October 29. 2018, front parking lot, an employee was not required to wear a lanyard attached to the boom or basket when he moved the aerial lift basket to or from the building roof. The employee was exposed to falls greater than 20 feet to a lower level.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall protection system:         a.  On or about October 29. 2018, office building roof, an employee was not protected by a guard rail system, safety net system, or personal fall protection system when working at the roof.  The employee was exposed to falls greater than 20 feet to a lower level.
