On or about October 25, 2018, in the VFD Building at the Oryx Lease/Pyote Truck Station off Hwy 2355, employees were exposed to electrical arc flash hazards as the employer failed to verify the effectiveness of lock out devices and did not ensure that all components had been tested and locked out with energy control measures before allowing employees to work on reconnecting electrical cables.
