29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At the jobsite, employees worked under other employees at lower levels without wearing hard hats exposed to head injuries from falling objects.  Raul Sandoval Construction, DBA M2W Xcavation Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets, which was contained in OSHA inspection number 1339270, citation number 1, item number 1 and was affirmed as a final order on September 24, 2018, with a final abatement date of September 25, 2018, with respect to a workplace located at 2351 Arkansas Lane, Arlington Texas.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  At the jobsite, employees worked from a tubular welded frame scaffold that was not fully planked on all working levels and were exposed to the hazards of falling. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  At the job site, the employer did not provide a scaffold ladder where the employees could access and egress the tubular welded frame scaffold exposing them to fall hazards. 29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  At the jobsite, employees installing masonry material from a (3) tier tubular welded frame scaffold without guardrails were exposed to fall hazards of approximately 19 feet to the ground below.
