29 CFR 1910.147(c)(4)(i)  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about 10/26/2018, at the Compounder Area, southwest corner,  the employers' Lock Out/Tag Out written program did not contain machine specific procedures for the TPU Hopper #3 Discharge Unit, exposing employees to amputation hazards.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources.  Lockout/Tagout hardware (cable) was not provided or used to secure the pneumatic slide gate during clearing out of bridged material operations of the TPU Hopper #3 Discharge Unit, exposing employees to amputation hazards.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:  On or about 10/26/2018, at the Compounder Area, southwest corner, an employee had not received appropriate training on lockout/tagout procedures  on,  de-energizing the pneumatic slide gate prior to properly clearing out bridged (jammed) material from a support hopper in the compounding process.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.  On or about 10/26/2018, employee(s) engaged in various operations to include, but not limited to compounder,  had Forklift Operator Certification that had expired.  Employers inventory consist of  approximately twelve trucks, to include but not limited to, Clark Model #TCX25, Serial #TMX250 250 2820 9803 KF.
