29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about October 29-30, 2018: At the Prima #1 machine infeed section door and the shear press back door guarding were not effective since the interlocks safety switches were disconnected which allowed the operator to run the machine with doors open; exposing employees to amputation hazards. b) On or about October 29-30, 2018: At the Prima #2 machine the infeed section door was broken and the shear press doors guarding were not effective since the interlocks safety switches were disconnected which allowed the operator to run the machine with doors open; exposing employees to amputation hazards. c) On or about October 29-30, 2018: At the Bravo #1 machine the stitcher door, infeed section doors, shear press doors guarding were not effective since the interlocks safety switches were disconnected which allowed the operator to run the machine with doors open, and shear press back door guard was broken; exposing employees to amputation hazards. d) On or about October 29-30, 2018: At the Bravo #2 machine the stitcher door and shear press doors guarding were not effective since the interlocks safety switches were disconnected and allowed operators to open the doors while the machine was in operation; exposing the operator to amputation hazards.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: a) On or about October 30, 2018: Two bottles were found near the Prima #2 machine area that employees use to lubricate the machines, the employer did not ensure that each secondary container of hazardous chemicals were labeled, tagged or marked in accordance with OSHA standards and exposed employees to skin and eye injuries. Trend Offset Printing Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(f)(6)(ii), which was contained in OSHA inspection number 1276885, citation number 1, item number 1 and was affirmed as a final order on December 29, 2017 , with respect to a workplace located at 10301 Busch Drive North Jacksonville, FL 32218.
