29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    (a)  An employee working from a 6:12 pitch roof while conducting dryer vent exhaust cleaning was not protected from falling to the lower level. This exposed the employee to a fall hazard of approximately 8.8 feet.

29 CFR 1910.30(a)(1): Before any employee is exposed to a fall hazard, the employer did not provide training for each employee who uses personal fall protection systems or who was required to be trained as specified elsewhere in this subpart.    (a)  The employer did not ensure that an employee exposed to a fall hazard had been trained to recognize and minimize the fall hazards associated with his work.
