29 CFR 1926.404(b)(1)(i):  The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:       a)  On November 1, 2018, employees were provided with electrical tools and equipment connected by cord and plug without the use of an assured equipment grounding conductor program or ground fault circuit interrupters (GFCI's).       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:     a)  On October 29, 2018, employees were not provided with a safe point of access where there was a break in elevation of 34 inches between the bottom step of the stair case and the ground.    No abatement certification or documentation is required for this item.

No abatement certification or documentation is required for this item.
