29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) At 21 Parks Edge, Orange Beach, Alabama: On or about October 30, 2018 and at times prior; the employer allowed his employees to perform siding activities from a pump jack scaffold without fall protection exposing his employees to a fall hazard of 14.9 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 21 Parks Edge, Orange Beach, Alabama: On or about October 30, 2018 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the ladder jack scaffold platform and the rails of the ladder did not extend three feet above the platform to provide safe access exposing his employees to a fall hazard of 14.9 feet to the ground below.
