Astoria, OR. 97103 - On February 2, 2019 Integrity Structures, LLC. failed to provide Abatement Certification for Citation 1, Item 1 issued on January 22, 2019 from inspection #1356758. The final order date for the original citation was set on February 15, 2019.

29 CFR 1926.153(o): When damage to LP-Gas systems from vehicular traffic was a possibility, precautions against such damage were not taken. (a) On or about October 30th, 2018 and at times prior thereto, employees are exposed to fire and explosion in the event a vehicle makes contact with the LP Gas tank that was not protected from vehicular traffic. Integrity Structures, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.153(o), which was contained in OSHA inspection number 1120502 , citation number 1 , item 1 number and was affirmed as a final order on 03/07/2016, with respect to a workplace located at 200 W Klaskanine Ave Astoria, OR 97103 .
