29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about October 30, 2018, the line 1 Uniloy Blow Molder was not adequately guarded, in that, employees were exposed to a caught-in hazard with the swing arm while reaching through the approximate 4-inch gap in the guarding to clear bottles that jam in the machine. b) On or about October 30, 2018, the line 3 Uniloy Blow Molder was not adequately guarded, in that, employees were exposed to a caught-in hazard with the swing arm while reaching through the approximate 4-inch gap in the guarding to clear bottles that jam in the machine. c) On or about October 30, 2018, the line 4 Uniloy Blow Molder was not adequately guarded, in that, employees were exposed to a caught-in hazard with the swing arm while reaching through the approximate 4-inch gap in the guarding to clear bottles that jam in the machine. d) On or about October 30, 2018, the line 5 Uniloy Blow Molder was not adequately guarded, in that, employees were exposed to a caught-in hazard with the swing arm while reaching through the approximate 4-inch gap in the guarding to clear bottles that jam in the machine.
