29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a.  During the unjamming of a Switchback box forming machine an employee did not apply energy controls by; turning off and/or shutting down the press [per the 1910.147(d)(2) requirements], isolating all energy sources [(d)(3)], affixing a lockout device to each energy isolating device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to unjamming the machine [(d)(6)].      b.  During die change operations of Switchback machines employees do not isolate all energy sources [per the 1910.147(d)(3) requirements], affix a lockout device to each energy isolation device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to conduction a die change on the machine.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   On or about October 11, 2108, the employer did not ensure one or more methods of machine guarding was adequately provided to protect machine operators from the hazards created by the point of operation of the of the Switchback box forming machines.  An opening in the underside of the front door guard exposed machine operators to moving machine parts of forming punches, forming dies and vacuum pickers.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.     On or about October 11, 2018, the employer did not conduct periodic inspections of the energy control procedures to ensure machine operators performing service and maintenance operations, including but not limited too; die changes and clearing jams, on the Switchback box forming machines were following the procedures and the requirements of the standard for energy isolation and control.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    On or about October 11, 2018, the employer did not ensure a machine operators required to perform service and maintenance activities, including but not limited too; die changes and cleaning jams of Switchback box forming machines were trained in the type and magnitude of the energy available in the workplace and methods and means necessary for energy isolation and control of the box forming machines.
